Preparing for Surgery
To expedite your check-in process, please log onto One Medical Passport and opt into text and email communications, then complete your health history and registration information prior to your scheduled procedure date. We need to receive this information 24 hours prior to your procedure date. If not received, the Pre-Registration Department will be contacting you to complete the required information over the phone. If you should need assistance, please contact the Pre-Registration Department directly at 610-337-2828 ext.223.
A Health History Nurse will call you prior to your surgery to collect any additional information if necessary.
It is important for you to follow these guidelines before surgery to prevent delay or cancellations.
